How do I check an Ohio assisted living license myself?
Search the Ohio Department of Health's provider lookup by name or State ID; the record shows license status, the current license period, licensed capacity, the operator and the date the facility first opened.
Each State ID in the table links to that community's page in Ohio's Long-Term Care Consumer Guide, which lists every ODH licensure and complaint survey for roughly the past four years with the number of citations found. What do Ohio inspection records show across Columbus-area communities?
The median Columbus-area RCF received 1.7 citations per ODH licensure survey, and none of the 116 records shows a complaint survey flagged for substandard care or immediate jeopardy.
Citation counts are a starting point, not a verdict. A large building is surveyed against more rules, and a single survey can produce several minor paperwork citations. What is Ohio's resident satisfaction survey?
The Ohio Department of Aging interviews residents in person about care, caregivers, meals, activities and moving in; 50 of the 116 Columbus-area communities have a published 2025-2026 report, against a statewide average overall score of 85.5.
Communities with too few eligible or responding residents get no published score, so a blank is not a bad sign by itself. Many facilities also have an older 2021 family satisfaction report. The full profiles show both, with dates, and we never turn them into star ratings.
